I noticed that helicopters are usually placed right on the helideck but sometimes they are depicted flying over the ship. I'd guess the latter case is for ships that do not have a hangar, is it correct? In general, is there some kind of rules or guidance on placing helicopters? Place em on the land...

It is, but only as long as the other guy is capable of threatening you on the same level. Which kinda rules out most nations, and makes the Ohio's useless in the vast majority of conflicts.
OHPs can't take a VLS array where you put one on the New Paisley Class Destroyer.
Eh? Maybe not a huge one, but there's room for a VLS allright. I mean, the MK-13 sits there in an ordinary OHP, that thing takes up quite a bit of space as well.
