I know in peace time hospital ships are not ''allowed'' to be armed even for self defence, but in a war situation (a war like the WW2) are these ships to be armed or they will get escorted by other naval ships? And if it would be armed what would it be? phalanx or Searam or an other system?

According to the Hague convention of 1907 Hospital ships are un-armed. They have to be painted white carry the Red Cross and during the night have to show lights. They cann't carry troops, only sick and wounded.
During WW2 both sides attacked Hospital ships, the most famous incident the sinking of the Australian Hospital ship Centaur
